On behalf of the Program Leaders Committee at the Fetal Heart Society, we invite you to participate in “Fetal Cardiology Benchmarking Survey” (one entry per program). This is an international survey of academic as well as non-academic practices that offer fetal cardiology services to help understand current trends and practices in fetal cardiology.
Your participation and the information you share will be critical for programs to plan appropriate resource allocation, design QI initiatives, and advocate for an adequate workforce. Your contribution will allow you the option to be be acknowledged in the future publication of the survey findings.
We request that ONLY ONE representative from your Fetal Cardiac Program completes the survey below. This survey can be completed either by the Program Director, QI leader or representative assigned by the program director. We are only able to accept one entry per program and will acknowledge the first person to respond to the survey in the resulting publication.

This survey will take approximately 10-15 minutes to complete based on the program size.
